Copyright Claims

At Alaska Forms, we hold intellectual property rights in high regard and adhere strictly to the laws governing them. It is our policy to respond to any claim that Content posted on the site infringes copyright or other intellectual property rights ("Infringement") of any person or entity.

Submitting a Claim

If you believe that your copyright or other intellectual property rights have been infringed by content hosted on our site, please provide the following information in a written notification to our designated Copyright Agent at dmca@alaskaform.com:

  • A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
  • Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, along with sufficient information to allow us to locate the material.
  • Information reasonably sufficient to permit us to contact you, such as an address, telephone number, and, if available, an email address.
  • A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
  • A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Our Response

Upon receiving a proper infringement notification, Alaska Forms will take the necessary actions to remove or disable access to the infringing material and will make a good-faith effort to contact the owner of the removed or disabled content so that they may make a counter notification pursuant to sections 512(g)(2) and (3) of the DMCA. Furthermore, we may document notices of alleged infringement on which we act.

Counter Notification

In instances where your content has been removed or access to it has been disabled, and you hold a belief that this was done in error or that your content does not infringe upon any copyright, you may submit a counter-notification. This must include your full name, address, phone number, a statement that you consent to the jurisdiction of the Federal Court for the judicial district in which your address is located, or if your address is outside of the United States, for any judicial district in which Alaska Forms may be found, and that you will accept service of process from the person who provided the original infringement notification or an agent of such person.

The counter-notification should also include a physical or electronic signature, identification of the material that has been removed or to which access has been disabled, and a statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of a mistake or misidentification. The counter-notification should be sent to our Copyright Agent at the email provided above.
